I want to select an appointment in the calendar and have it show up as being selected.  When I use ItemSelection.Add(app) the item is selected so I can see it in code but it visually does not appear selected to the user.  How do I do that?

Adding the appointment to ItemSelection should be enough for it to become visibly selected. What is the theme you are using? In any case, make sure that the item has a distinguishable style for its selected state.
